Date: 21st September 2005 Key Events An under ground cable of Telecom Malaysia was damaged during an excavation work at Sungai Choh, Rawang on 30th November 2004. As the incident was not reported to Head Office, as such no notification was done to inform the insurance company pertaining to the accident. A police report was lodge, but was not forwarded to Head Office. Due to the above, the company may have to settle the repair cost totaling RM 22,343.57 by its own. Critical Knowledge More serious precaution action should be taken during any excavation work. All accident must be reported to Head Office within 24 hours. All relevant documents such as police report, photograph must be forwarded to Head Office for necessary action. The company lost RM 22, 343.57. Key Lessons More serious precaution action should be taken during any excavation work. All our property including third party property in our working area are insured, as such any damages or accident are claimable from the insurance company. Should the accident was reported to Head Office, the company may not have to bare the repair cost amounting of RM 22,343.57. Proposed Action Proposed to obtain an approval from the involved utility company prior any excavation work are being carry out. Regional Managers, Deputy Regional Managers & Area Managers are to ensure Head Office are been notified for any accident occurred. A serious disciplinary action should be taken against the responsible person should they failed to take appropriate action in notifying Head Office for such incident.
